When I'm watching, I see a lot of these calls when the tackler (usually a DB or LB) lowers his head and then the ball carrier lowers his head and/or body and head to head contact is the result and the tackler gets booted. First of all, they have to PREACH not to lower your head to lead with your helmet. The spirit of the rule is to prevent injuries, the execution of the rule has lead to some senseless ejections.

Head to head collisions happen on every play at the los. Linebackers blitzing, db's gapping on run support. O-lineman & D-lineman spend the entire game slamming into each other with their heads.
